I have been using Dot Net Nuke, after being introduced by Medical Objects.
We have used this as a collaboration tool for the Interoperability Demo 2006
(next week at HIC conference).

It has the features like hierarchical permissions etc.  and runs on MS
platform as an open source development.

Worth looking at. Glenn Stephens is the Medical Objects guru on this tool.
