There is a GNU licensed program for appointments/demographics using a LAMP setup
which links with the SQL Ledger software.

It is not clear from the site what the licence is - I have queried them re this and the cost of customization. Uses PERL with PostgreSQL backend - web based - there are demo sites which appear to run quickly with the amount of data present (smallish samples) Looks like it would be a fair bit of work to get it up to speed, but most would be in getting the detailed user requirements and workflow of each process, and the reports required and then customizing
