Agree - appointment type has no relationship to provider
The provider table has a link to appoint type - presumably this is his default. Is this really necessary?
If he has a default_type perhaps this should be a separate data item

re the appointments tables, why is there a healthcare_provider_id on appointment_type ? Appointment_type is supposed to be A, B, C and has has a duration attached to it, so maybe it should be appointment_duration_type, and appointment_category could be appointment_service_type (pap smear, procedure, health assessment, medical report, gp medical management, etc) , to be less ambiguous in naming .
