I think all you're telling us is that different governmental and health system structures facilitate this sort of infrastructure easier than others. In Australia we have had the spectre of a now state premier offering to hand NSW Health over to Canberra, only to be rebuffed.

Never forget that both NZ and Denmark are both much smaller, less complicated political and governmental entities.

I'm not sure we are any further advanced in thinking about what will work for us with States, Territories and Federal government involvement.

The Eastern Goldfields site shows government can do anything it wants on a small scale if it is prepared to pump in enough money, but that isn't going to happen on a big scale anytime soon. They don't believe in public infrastructure, and won't either fund the states or do it themselves.
