Tim et Al, I fully agree with Andrew S. here. Some people are so keen on an idealistic model of healthcare IT delivery that they blithely overlook the fact that the only environments where effective integration in primary care exists is where there is an integrator such as Argus, Medical Objects or HealthLink knitting the sector together. Without a party working to pull things together it (Primary care integration) just doesn't happen.
I am in near daily contact with people all over the world and I can assure you this is a sound assertion. Please do tell us of one environment in which clinical applications "talk" electronically on any scale without an intermediate system or systems involved. The only one that has a hope of doing it is the UK with its centralist command and control approach and x billion pounds expended and counting. However even with all that expenditure, the UK is still way behind other countries (that have a more pragmatic approach) when it comes to connecting primary care. I can assure you that integration models such as that provided by Medcom in Denmark and other examples closer to home are much more cost effective, responsive and indeed highly successful. Considering how hard it is (and expensive) to get them to make the smallest change to their systems, I just don't see how this can ever be accomplished. Mindful that we should be ever careful of what we wish for, I advise against wishing the message vendors away...
