Does anyone have experience in digitally signing a referral when
connecting to a host over a Citrix or Terminal Server remote access
session? Can this be done (using a HESA key)?

Experience? No sorry.
Is this possible with HESA keys? I believe so.
A remote access Terminal Server session should be exactly the same as any LAN
based Terminal Server Session. If this can be done from within the LAN then it
can be done via remote access.
HESA provide two options for their keys.
1. is the rainbow i-key USB dongle that most seem to have.
2. is the smart card.
The smart card option can be also be used by windows server 2003 to
authenticate users for Terminal Services Sessions.

Not tried this as it uses Active Directory (AD) which I don't use. My
expectation is that a user who authenticated with a smart card could use the
same credentials to digitally sign and encrypt e-mail.
The use of personal encryptions keys doesn't sit well with me in a group
practice situation - I'm more in favour of practice keys.
