For unnamed episodes, the current way it works is that their publication date
is set to the day when the first episode status for it is uploaded. This can
lead to problems with very old episodes which are available to the client, but
not the server when it fetches the RSS feed.
I propose that all "unnamed" episodes are moved to a separate section to not
"pollute" the normal episode list (with known-good metadata).
For the overview, especially the "Last episode:" info in the "my subscriptions"
view, it should also prefer known-good episodes (with a title) instead of
"Unnamed" episodes, because "Latest episode: Unnamed episode" is not as
informative as "Latest episode: <name of previous episode>".
