That is in header and footer of the page style. Double-click on "default" in the status bar. That opens the property dialog of the page style. Or use F11 to open the styles and formatting window and switch to Page Styles there.
On the tab Header or Footer respectively click on Edit. There you can erase the table name and page number. You only need to click in the area field and delete the entries.
3. I forgot to add a y-axis label and can't figure out how to add one now.
Double click the chart to bring it in edit mode (gray border). Then in the menu Insert click on item Titles.
