Werner, the plugin sends data in constant intervals. The 'run_rate' defines the seconds between each update. So in the case of message backpreasure, there will be no metrics spike. But, only the processed messages are reflected in the Graphite graph, messages waiting in the buffer cant influence the graph. So the graphs will maybe slightly flaten during peak times. Hope that explains the mechanism.
