repeating:
I found that I ran into this problem when I had calls to:
GM_registerMenuCommand(null);
I had used this for spacers. But at some point, a new version of GM
broke on those and created that same weird little menu. I find that
now I have to use:
GM_registerMenuCommand('', function(){});
