Have you tried a fresh profile? If you see it after installing
Greasemonkey into a new profile then it is likely an
installation-specific issue with your current settings (eg,
conflicting addons) (which may/may not be caused by a bug).

I've no idea if this is actually the case but it seems like there was
an error during installation - Firefox believes the icon is in the
toolbar so doesn't display it in the customise dialog perhaps?

Have you tried uninstalling then reinstalling Greasemonkey perhaps?
