If you mean http://icanscale.com/callout/ it's been quite a few years since that was changed so it's not super surprising that it's broken. It seems much more likely to me that it's the source of the problem. The AMO link just says it's been disabled. Because it's broken? Or unsafe? Its install.rdf says max compatible Firefox version is 3.5.
