I think (I haven't really tested) this might be fixed using the latest  
version of fontspec, which is not on ctan yet. When it is, you can use  
tlmgr update to update the packages..

Try using (the font names):
\setmainfont[Ligatures=TeX, BoldFont=GaramondPremrPro-Smbd,
              ItalicFont=GaramondPremrPro-It,  
BoldItalicFont=GaramondPremrPro-SmbdIt]{Garamond Premier Pro}

You should not need to do any redefining - that's why we suggested  
using fontspec ;)
For instance, after using the above command in your aehymn.tex,  
greitalic is now 12pt GaramondPremrPro-It , gresmallcaps is the small  
capitals from GaramondPremrPro, grebold is 12pt GaramondPremrPro-Smbd.

But say, for example, you wanted the italics in your score to be a  
different size from the normal text? Then you could say

\def\greitalic#1{%
{\fontsize{8}{8}\selectfont \textit{#1}}%
}

Which would print smaller italics (in 8pt GaramondPremrPro-It) in your  
score.

No, it's ok! I think a lot of people on this list are not used to TeX  
in general, and benefit from the questions.
