I wrote a tool to do this last year.  I wrote it as a webpage, which you
can access here: http://jgabc.christian.net/transcriber.html.  If you use
the web browser Chrome, you can also download it as a web app here:
https://chrome.google.com/webstore/detail/mkhgaijffgijgkolifpoaijimncdakkd.

Enter the text of the hymn on the left side without any parenthetic GABC
notation, and then enter all the tones on the right side with each syllable
separated by a space and no parentheses.  The example hymn should be
helpful in demonstrating how the tool is meant to be used.
