I can assure you it works very well under Lion.
Part of your trouble is that Apple recently changed the way Xcode is installed.
It's a good news/bad news sort of improvement. To get to gcc itself, you'll
need to install the Xcode command line tools. Launch Xcode, open the
preferences, go to "Downloads" and install the command line tools.
It's free, but you will need to create an AppleID/developer account. If you
already have an AppleID (which you must since you installed Xcode), just use
it. Xcode itself will walk you through.
Once that is installed, you can compile and install gregorio.
Which MacTex live are you using?
