[mm]: Improve handling of `CS` macro.
* contrib/mm/m.tmac (CS): Define to throw diagnostic when used without
".MT 4". (Later, if the "4.MT" file is sourced, this definition is
replaced.)
* contrib/mm/groff_mm.7.man (Description): Clarify `CS` support status.
