Growl only allows one or the other, since it implements speech as a display. So, either you're seeing Skype's built-in visual notifications and using Growl's Speech display, or hearing Skype's speech notifications and seeing one of Growl's visual displays. If it's the former, turn off the Skype notifications you don't want in Growl's Applications tab.
