i got no help from mozilla either. but after a lot of testing, i
figured out it was the "download status bar" addon even though i'd
uninstalled it. just in case anyone else comes looking here with the
same problem:

"download status bar" disables growl notifications (see growl.info)
for "all downloads complete" on firefox 3.6.13 on the mac. (don't know
about windows).

notifications remain disabled if you disable download status bar in
the addons window.

if you then uninstall download status bar in the addons window, growl
notifications remain disabled, permanently.

the workaround is to re-install download status bar, enable it, and
then uninstall it. in other words, if you uninstall download status
bar it while it's disabled, it won't be properly uninstalled. it will
prevent growl notifications from working. it must be enabled before
you uninstall it.
