That's the first thing to check. Try the Simplest Notifier Evar:

If that works, the problem is with Sparrow, and you should contact Sparrow's
developers.
If even the Simplest Notifier Evar cannot get a notification through, the next
step is to open up the Growl preference pane.
Does the button say “Start Growl” at first?
If so, does it change to “Stop Growl” after a second or two?
(After you determine that, quit System Preferences.)
If it does, do notifications work after that? (Try the Simplest Notifier Evar
again.)
If so, does Sparrow re-break them when you relaunch it?
