If a license conflicts with Mac App Store terms, then there are two ways the 
app can get excluded:

1. Apple takes notice and removes the app. (So far, I'm not aware of this 
having happened… yet.)
2. Somebody who contributed to the project takes notice and sends Apple a DMCA 
takedown demand, and Apple removes the app. (VLC)

I haven't read the LGPL myself, but if what Chris says is correct, that Apple's 
usage restrictions conflict with a provision in the LGPL (as they do with a 
provision in GPLv3), then all OsiriX's presence in the App Store means is that 
nobody has noticed, or nobody cares about, the conflict.

If a conflict exists, any app published under a conflicting license—or any app 
that includes code published under a conflicting license—has its days numbered 
in the App Store. Either Apple will remove the app on their own (if they decide 
to start doing that) or somebody will demand that they remove it.
