This hasn't changed since 2004, other than how you get GrowlTunes. 

GrowlTunes needs to be added to your login items. Just like any other software 
that you want to run at startup. We're working on adding an option into 
GrowlTunes 2.0 so that it's a simple drop down, like with Growl, 
HardwareGrowler and Capster. However, since we haven't released GrowlTunes 2.0 
yet and you're using 1.2.2, you're going to need to do the same thing that 
everyone else who has ever run GrowlTunes will need to do in order to get it to 
run at login.
