I've had the same problem since mountain lion, Hardware growler and growl tunes all start up just fine, but the main Growl client struggles. Even when bypassing both growl and the Login items with Growl manually added and running a startup script it still misses sometimes.
Hopefully the Dev team gets an update out soon.
