Im not sure exactly what would be causing issue one, although unlike the icons for other monitors, we get those from the system (and yes, I know the system has larger versions, for some reason we aren't pulling in a sufficiently sized version). In general, its best to go ahead and report bugs rather than assume something will be fixed, as often times things slip through the cracks.
I know part of the reason for 2. It was changed because the old white icon looked terrible if you used any white background display style (it basically disappeared). Perhaps different colors might be better, but thats not my call. 3 should fixed for the next release of HWGrowl, we had a capitalization issue in the file names for the icons. If you are running on a case sensitive file system, that would likely explain it. In theory you could fix it by renaming the images in the app bundle's resources from Firewire-On and Firewire-Off to FireWire-On and FireWire-Off (modifying bundle resources not really supported or encouraged though as it might mess with things like the bundles signature).
