My guess is that some piece of the system has cached the wrong size of
the icon, causing it to display in launch pad fuzzy.  Finder/Spotlight/
Launchd or some combination thereof.  Finder and spotlight can
generally be resolved by relaunching them, launch services would need
its database built, which can be done using a utility such as Cocktail
or via the command line.
