The myproxy-logon -d option isn't "debug" but "dn_as_username", and I don't think that's what you want. (Sorry that -d means different things for myproxy-logon and myproxy-server.) Using just myproxy-logon -v -s localhost should work better, so hopefully the corresponding output from 'myproxy-server -d' will identify the problem.
