QUEUE should also be sent when the queued host fails to reissue
a request within the maximum specified time, plus a margin of ~30 seconds.
The remote host might have crashed, and if we are firewalled, he won't
be able to recontact us.
We could limit the scope of QUEUE (i.e. be rather strict on which cases
we want to cover), but I wonder whether we should? It might be best to
blindly issue a QUEUE whenever the request did not come in time. Even if
we aren't firewalled.
