I answered my own question in the end but, in case anyone else has the same problem:
Gtk+ provides quite a lot of flexibility for custom theming engines to choose how they render widgets. Thus, if you don't explicitly specify which theming engine you want Gtk to use to draw your widget with (e.g. with the css "engine: none" tag to tell Gtk to use the inbuilt engine) then you can't guarantee that it will look a certain way. This comes with the usual caveat that it's generally more important for an application to fit in with the rest of a desktop theme than for it to look exactly a certain way.
