I am using
http://ftp.novell.com/pub/mono/gtk-sharp/gtk-sharp-2.12.10.win32.msi, 2.12.9
also works. 2.12.8 and back had problems with the theme, but calling
System.Windows.Forms.Application.EnableVisualStyles fixes it (it doesn't
matter that this method is part of WinForms, it works). However, I'd
recommend using 2.12.9+, they don't need this tweak.
