I can't help on the custom front, but there was a dealer rep from a new company called AFX helmets at Parry Sound. I was inquiring about larger sizes since both my Shoei RF800 and Arai Quantum E are size XXL. He said they make they up to XXXXL. In checking their website http://www.afxhelmets.com it looks like the 4Xl fits 68-69 CM, so you might even fit in a 3XL.
