At the GM's whim. Possible way for them to introduce a new world include:

* Natural portals
* Dimensional highways
* Mysterious travellers or artefacts from the world.
* Offers of knowledge from higher (or lower) powers.

The setting says it's designed for a fairly small set of worlds. If
you want to have a lot of them, adding an astral plane that lets you
travel between many, many worlds is an easy way of doing it. But it's
really hard to run a campaign that has thousands of worlds, just
because it's hard to make up so many.
