There's an entire wiki on this, along with many web sites, FAQs, and
"how to" documents.
 
My favorite BitTorrent client is UTorrent (or sometimes called
Mutorrent).  You used to be able to get it from http://www.utorrent.com
 
 
How to use a BitTorrent client obviously differs from one to the next,
but this one is very screen reader accessible.
 
The idea is that someone who has a large file they wish to distribute,
say the music cd of their own original music they wish to give away,
would first find a torrent site which has a "tracker".  This is a web
site running a special piece of software to allow it to connect those
who have torrents with those who want them.  There are many such free
torrent sites.
 
They then create a very small file called a torrent file, which
describes their music cd file in technical details, and they upload this
torrent file to the hosting site.  It's usually around 1k in size.
 
This person goes on to load a BitTorrent client program on their pc, and
they tell it that they have this cd file they will share (called
seeding).  Their client tells the "tracker" that they're ready to seed.
 
Other people may download the small torrent file, which points them back
to the "tracker".  These other people run their own BitTorrent client,
give it the torrent file, and it contacts the tracker and says "who has
this".
 
The tracker gives their client IP addresses of other users who are
willing to "seed" this file, and your clients begin exchanging pieces of
the file.  
 
your client may contact many such users, each of which may send you a
piece of the file, all coming in at once and being put together by your
client.
