There are commercial programs such as Omnipage that can read PDF's, but they
are doing this as a sideline to their primary function of OCR.  I think you
should get accustomed to the idea that Adobe Reader is a growing program (30
MB or more), because when there is text in a PDF file, it reads it with
several options.  I have also used the feature for commenting on a PDF
document, creating a sort of "post-it note" that is attached to a page which
I send back to the author.  Adobe Reader is the only program that sends the
text through MSAA.  Some of the others probably only create a nice display
on the screen which is one large graphic that screen readers cannot
interpret.  

I do sometimes use the PDF features of Omnipage to get a different
perspective on the layout of the pages, because Adobe Reader doesn't always
get these correct.  But most of the time, Adobe Reader is all I need to
understand a PDF and to save a text file from it.
