Hello Karyn. For the most part, the Adobe Reader access has not changed.
So while Window-Eyes 5.5 is ages old, it may still work reasonably well
with Adobe Reader 9. My guess is that the form is either a secure pdf,
or a scanned image like you suggest.

First of all, your WE version might be a problem here. Someone else would
have to answer that one but it wouldn't surprise me if that were the case.
The second thing I would check is whether or not the document is a scanned
image. If it is, screen readers don't like these things and you would have
to run it through a scanner program such as Kurzweil 1000 and that doesn't
always give you a great rendering of the document either. I did one of
these recently and didn't get a great rendering but got enough to know what
the document was saying. This was under Adobe 8 on a Vista Home Premium
machine running IE 7.
