All of the menu items you've listed are due to bugs on the Mozilla side of things. I believe we've either reported these issues, or are in a dialog now where we can discuss them.

Subject: WE 7.1 beta 1 and Firefox 3.10 and 3.11 menus

I am currently using the Window-Eyes beta with Firefox 3.11 on a
Toshiba laptop with Windows XP SP3 as the OS. I began noticing the
following issues while navigating the Bookmarks menu, which I use
extensively. Initially, I wondered if these behaviors were occurring
because my Bookmarks menu is fairly large and contains several
pulldowns. However, I discovered they occurred in all the other menus,
too.
These things don’t happen 100% of the time, but they are quite frequent.
1. If alt plus a hot key is pressed to access a specific menu,
Window-Eyes might announce, “Context Menu,” before announcing the menu
item with focus.
2. In this same situation, Window-Eyes might also simply announce,
“Context menu,” without indicating the first item is in focus.
Pressing CTRL+Center, at this point, causes Window-Eyes to announce,
“No menu item selected.” Pressing Enter activates the seemingly
“unselected” item. Also, my husband confirmed the menu item was indeed
highlighted.
3. Opening submenus with either a letter or the right arrow key may
yield the same results described in item 2.
4. Pressing Alt to move the focus to the menu bar may get an
announcement of “Context menu.” In this case, however, a press of
CTRL+Center causes Window-Eyes to indicate the focus is on the file menu.
5. In the process of trying to slow down and see if I could work
around the issues described above, I discovered that pressing and
releasing the Alt key and then pressing a menu accelerator very often
caused Window-Eyes to announce the letter pressed but not the focused
menu item. Pressing CTRL+Center caused Window-Eyes to state that no
menu item was selected.
