Yes, the server itself need to have a screen reader. The concept is pretty simple, remote access is like walking to another pc or workstation, if it does not have any screen reader on, you can't expect to get much work done for a blind folks. Of course you can use alternative like narator.
As for VNC, for my opinion, its much more responsive and customizable as compared to the native of RDP and it support higher encryption. But unfortunately, they don't support audio streaming or screen reader like RDP does. >From experience with VNC, I connect to my friend's pc, then I call them and hear what I do on the phone.
