Window-Eyes will not work in safe mode regardless if you have an external synthesizer or not. Like the previous poster said, Windows only runs the critical drivers for it to run and nothing else. But I agree that it should be able to run in safe mode just in case you needed to restore your system and you're totally blind. Or something else.
