If you are using a standard headset mic combo, ie, not a usb headset
mic, then it will depend entirely on if your sound card has the
ability to record it's wav, or stereo mix out-put.
Some sound devices won't do this.
Then you must look at possibly getting a mixer and another sound
device for the screen-reader, there are also virtual sound routing
utilities like virtual audio cable and such that can do this for you,
all the possibilities go beyond the scope of this email.
Look for something called stereo mix, or wav out, or something
similar on the input side of your sound card's control panel, or the
windows volume control
YOu must also make sure your recording software is set to that input source.
