Richard,

alt-f4 should close any application; I don't know why it's not working for
you, unless perhaps windows thinks your control key or your shift key is
stuck down (sometimes that happens to me); several taps on each of them
usually sets things to right.

whenever you close an app though, it doesn't mean you'll go to the desktop;
it usually activates another window, but if there isn't one, you may or may
not end up on the desktop.  to force yourself to be on the desktop, use the
windows key and D, or windows key and M (which also minimizes all windows,
so I think it's a better choice).
