This is something that I can produce rather consistently.
 
Windows 7 SP1 with three accounts, one administrator and two standard accounts. 
WE 7.5 set up to run at the log in screen, always run after logging in always, 
and show icon in the notification area instead of the taskbar.
 
When I press alt+F4 on the desktop then select log out, then sign into another 
account, there is always an error report to be sent to GW Micro.
 
Changing Window-Eyes start up options so that it only runs for specific user 
accounts, got rid of this problem.
