This can occur if you have the Window Maximizer app installed. Removing it will solve the issue.

Hi. My computer was trashed by a virus, yesterday. A good friend fixed
everything for me, but noticed that all my windows are maximized, when
Window Eyes is running. We couldn't figure out how to get this feature
turned off.. For me, the problem arises, when I minimize Outlook and go
to another program. When i close that program, I end up with Outlook
opened up. Where do I turn off this maximizing feature?
