Regarding issue 1 this happens to me as well. Carolyn mentioned a while
ago to hit the enter key once and up arrow once which seems to solve the
problem for me.

load NVDA without making any changes the message and NVDA reads
everything correctly. If someone can help with this I would greatly
appreciate it.
