The only thing I do (aside from removing any bundled extras like games, antivirus, etc) on a new PC is to change the machine to run with best performance. This disables extra visual animations and effects, and it also removes the desktop from the Alt-Tab list. This is, of course, purely my own preference and likely doesn't have any noticeable effect on Window-Eyes. The user account control is the feature that asks you whether you want certain programs to make changes to your computer. Again, Window-Eyes will work fine independent of the UAC, so it's up to you as to whether you want the feature active or not.
