In Outlook 2010 the way I open attachments is to open the message. From the body, press shift+tab. Then I use the left and right arrow keys to get to the attached file I want to open. Once there, I will press the enter key.

In the past, if I were on a message in the inbox with an attachment, I could hit the context key, and h and there would be an option to open the attachment without opening the message.
