Personally, I have had no request from Firefox, for any kind of survey. Not 
even after upgrading. Haven't heard from anyone else, who has experienced such 
a request. Hence, my guess is, that it could be something else going on.

Does this happen at any given time, or, is it only when you start Firefox. If 
it is only at startup of Firefox, I would have guessed that you accidentially 
have changed your homepage to some kind of a website. That even could have been 
done by some software, that would have glitched your security package. 

If it is a changed homepage, open Firefox and hit Alt-T. Then arrow till you 
get to the Options, and hit Enter. Left arrow, till you get to the General tab, 
and tab down to the homepage settings. Choose something like a blank homepage. 
In the edit box, enter the following:
    about:blank
and then tab down to, and hit Enter on, the OK button. Close and restart 
Firefox, and see if you still would get this kind of request.

On the other hand, if the request does pop up even throughout your 
web-browsing, or at any other time and place on your system, I'd say it sounds 
a bit suspecious. Scanning your computer with some more aggresive antimalware, 
could then be suggested. Btw, I scanned my system with Malwarebytes a couple of 
days ago, with one hit - which I removed. Today, I updated the database, 
scanned the computer again, and it found three new instances of malware. So, 
maybe an extra scan with your software, could be worth a try. And, to give it 
the best go, close down any other software but Window-eyes, before you start 
the scan. If Firefox is open - for instance, I have seen cases where the 
antimalware cannot do its job on all the files included in the open software, 
hence you could have reports saying your system is OK, and yet it still would 
hold some troublesome things.

Well, this is all I can offer at the point. If the thing pops up at given 
times, or in the middle of given activity, let us know and it may be more 
obvious what is going on. And unless others here has experienced survey 
requests from Firefox, I would have deemed this to be useless. :)
