It depends on how you turn it on. If you set it to run at start up in
the Ease of access center then you have to go back there to turn it off.
If you just run it manually then just return to the Narrator window when
you're done, tab to Exit, do so, and it won't start up again automatically.

There should be a dialog where you can tab to "exit Narrator" then press
Enter.
You have to keep pressing alt-tab until you get to it.
