Yes, this is a long standing very elusive bug. I've experienced it, yet it's been a long time since I last did. And I have no idea what fixed it. For a while I suspected it was more likely to happen the more windows I had open at once. But that was based more on speculation simply due to the fact that the more windows you have open the bigger the juggling act becomes. And I'm not sure exactly what you're experiencing. Regarding clipboard text? For me it would be voiced every time I hit the control key. The remedy I finally thought of that worked for me was to essentially flush the clipboard. I'd pop up Notepad or anything else wherein I could enter a space, select it, and copy it to the clipboard. And that would solve it because there was nothing in the clipboard other than a single space character, which I do not have voiced. And as far as files being deleted in Windows Explorer? I experienced this too. But again, I haven't seen it in ages. So I can't even remember what I did. But it was either redraw the screen with Insert-Backslash or refresh the view with F5. Although I seem to remember sometimes having to back out of the folder and then step back in.
