I have encountered the same thing.
In fact, in Outlook WE just stopped responding but I did get it back up and
focus had changed from the window ehere I was reading text of a message to
the "From" field in the message headers. I was able to tab back to the
message body.
I have encountered something similar in using IE 11 with windows 8.1 and we
8.4.
A couple of times I found myself in the top stuff where the url and some
other IE related navigation  links are - don't know what that area of the
screen is called.
A few times WE just stopps speaking and I do manage to close the browser, go
to the Desktop and then reOpen the browser.
Also, often when typing in the text box to search for a ie 11 string in the
google accessibility page WE just wont read or speak anything inside the box
with browse mode off.
I have lost focus in a couple of other situations but these are the ones I
remember since they happen enough to register with my old brain.
Finally, if you have browse mode set to automatic you might arrow down over
a control and get locked into it with no indication other than you might
hear a sound to indicate WE has changed the browse mode.
Even when I have automatic browse mode turned off  often when I arrow over
some controls we will speak nothing for a couple, few, down arrow strokes.
Likely, this is a result of arrowing over some controls that don't have
anything WE can recognize as text for the interactive object.
Also, while down arrowing I quite often have focus stop reading, including
read to end, and focus placed at the top of the page. Sometimes focus will
return to where reading stopped but as often as not it jumps to the top of
the page.
Again, likely due to a page or something like perhaps a Ajax refresh or
something - don'tknow.
Like others I am looking forward to the next release of WE to see if any of
these situations improve stability.

Using WE 8.4 on a Windows 7 64 bit machine and using IE 11 and latest 
version of Firefox.
On numerous web sites the page is loaded and I start using the down 
arrow to read the page line by line.  After some random number of lines 
read correctly WE just stops talking.  Pressing the down arrow produces 
no speech but in some cases continuing pressing the down arrow does 
scroll the page on the screen but no speech.  Only way to get speech 
back is to close the browser and try again.  Going back to same web site 
may or may not read correctly.
