According to our Accessaphone contact:

"The only thing I can think of is the user does not have the patch applied. The 
latest version of accessaphone we have has been tested with the patch. Can you 
confirm the patch has been installed?"

So A) do you know what patch they're referring to, and B) do you have it 
installed?
