And, as people become more and more comfortable with touch
screening--pinching, swiping, and tapping, things are bound to degenerate
even further. Sad, but true.

You hit the nail on the head.  What Don didn't mention when he said he 
talked to me is I am able to duplicate focus issues on a machine that 
has never had Window-Eyes or any assistive technology installed on it.  
There clearly are focus issues within the operating system but as John 
pointed out, they just aren't noticed as much because they are pointing 
and clicking and that puts focus under the mouse. Few people drive 
Windows exclusively from the keyboard so they just aren't noticed.
