This is a known issue to Microsoft.
Here is a copy of my email to Microsoft:

Both Screen readers, Windows Eyes and Narrator, do not read the screen with mouse. Pressing window key I can arrow through the choices and the screen reader does verbalize the choices, but if I type anything, now arrowing does not verbalize the choices. Windows 10 is totally unusable for a blind user.

Many times a window pops up and all I hear when I tab is link link cancel, but what the link does is a total mystery.

In windows 7 when I encounter this problem, I can route the mouse to the link and the screen reader can read what is above or below the link to determine what the link is going to do; however, reading the screen with the mouse is not available in Windows 10. This is unacceptable.

Another issue, if I have several windows open, holding down the alt key and tabbing through the open windows gives no announcement as to which windows are open. Whereas, in windows 7, I hear thunderbird, firefox, skype, etc. when tabbing around.
